Having personally experienced the new birth (John 3:1-21, Romans 10:9,10) through faith in Jesus Christ, and having read the Local Church Constitution and By-Laws and the ‘Statement of Fundamental and Essential Truths’ of the Pentecostal Assemblies of Canada, I am in complete agreement with the practices and doctrines of Evangel Pentecostal Church (Québec). I would like to be associated with this church and its members in Christian fellowship and am therefore applying for church membership.

Requirements for Membership

A member is expected to give a clear indication of having been born again (John 3:1-21, Romans 10:9,10).
A member is expected to be baptized in water by immersion (Matthew 28:19).
A member must be 18 years of age or over for regular membership. (Junior membership is granted to those 12 to 18 who fulfill all other requirements, however, such membership does not qualify for voting privileges at congregational meetings).
A member is expected to live a consistent separated Christian life in accordance with the teaching of the Word of God (Romans 12:1,2), and to abstain from those things that are dishonouring to the Christian testimony of the Church (Galatians 5:24, 25).
A member is expected to be faithful in church attendance, and to support its ministries with regular tithes and offerings as the Lord shall prosper (Hebrews 10:25, Mal.3:10).
A member is expected to work for harmony and happy fellowship in the church, with the pastor, church officers, and fellow Christians at all times (Ephesians 4:31-32, Romans 12:5-18).
Automatic forfeiture of membership shall be expected at any time a member departs from the faith, ceases to live a consistent Christian life, changes his or her doctrinal beliefs contrary to our statement of faith, or fails to attend church without reason, over a period of three months
